We Welcome You!

Office Hours:  Appointments are available Mondays, Tuesdays, and Thursdays 11:00 am til 5:30 pm. Initial consultations are usually about 55 minutes allowing for a full review of history and labs. Follow-up appointments are usually in 2-3 weeks, depending on the number of issues/severity of problems and tests ordered. Follow-ups appointments are 55 minutes as well. Please be sure to bring any current lab work and have in mind the goals for your visit. Also, bring in all your supplements/medications for review.

 

Timeliness:  Due to the Doctors full schedule, we ask that you arrive at least 15 minutes prior to your appointment time, and come prepared with your history intake forms, your current medications/vitamins, and information ready.

 

Charges:  All consultation fees are payable at the time of the visit. We accept all major credit cards, HSA cards as well as Zelle.  We are currently out of network for all insurances at this time.  A copy of your superbill will be provided to you at the end of each visit to assist you in filing your own PPO insurance claim.  

 

Laboratory:  All patients are sent to the Lab Corp or Quest drawing stations most convenient for them. Many insurance companies require that you use a specific lab, and it is your responsibility to determine which is contracted with your insurance. Most labs are done 4 hours fasting midday. Female hormone testing by ZRT Saliva Tests are not contracted by insurance and must be submitted by an individual patient.  You will be required to submit any paperwork on your own if you desire to have your insurance cover specialized tests.

Appointment Courtesy:  We ask that you make every effort to keep your current appointment. It is difficult to fill such a large amount of doctor's scheduled time on short notice, and a rescheduled appointment may not be available for several weeks. As of January 1st, 2014 a "no show" or late cancellation fee has been implemented. Any appointment no-show or less than 72 hour cancellation notice will be charged a $125.00 fee to the credit card held on file. Monday appointments must be canceled by Thursday at noon to avoid a cancellation fee.

Refills: A patient must visit a minimum of once every six months, or depending on your conditions, more frequently, to refill your medications. Although most of these may be natural medicines, they still must be professionally monitored. Please call Strive Compounding Pharmacy at 619.270.8688 or schedule an office visit 858-663-7699.

Medical Records:  Lab results are provided at office visits. A full copy of your medical record is available upon request and signing of a consent form which authorizes the release of your records. A $25 fee may be charged for a full copy.

 

Telephone Calls:  Routine calls are taken daily during normal business hours, and every attempt is made to return calls within the same day. Prolonged discussions may require an appointment or phone consultation fee. Phone consultations may be appropriate in some cases & the fees are the same as office visits.  True life-threatening emergencies need to visit the E.R.

 

Arbitration Agreement:  All patients undergoing consultation and recommendations will be required, as of May 1st, 2008, to sign an arbitration agreement. Should a patient decide to decline the arbitration form, we will kindly refer you to another provider.
